Additional Information on Eligibility:

Any public or private nonprofit entity, including an Indian tribe or tribal organization (as those terms are defined at 25 USC 450(b)) is eligible to apply. Applicants must have at least four (4) years experience in the fields of quality management, quality improvement, developing and disseminating informational materials, and providing training or technical assistance related to the prevention and treatment of overweight and obesity on a national level. The Prevention Center may be a consortium of organizations composed of more than one eligible entity, but only one entity can be the official applicant for funding. All other organizations are members of the consortium or partnership. For-profit entities may be part of a consortium, but cannot be the grantee.

Description

The Health Resources and Services Administration Office of Planning, Analysis and Evaluation (HRSA/OPAE) will provide funding to support a Prevention Center for Healthy Weight (PC). The PC will plan, implement, and manage a nation-wide Healthy Weight Collaborative (HWC) as well as recruit and support communities and teams participating in the HWC. The PC will also serve as a gateway to quality information on the prevention and treatment of overweight and obesity in the context of integration of public and community health and primary care. This program seeks to provide and promote family-centered, community-based, coordinated care for children and families, and facilitate the development of community-based systems of services for such children and their families for the prevention and treatment of overweight and obesity.
